New Patient Paperwork

There are two steps to complete your new patient paperwork:

  1. Fill out the new patient paperwork (Packet) by hand or by typing in the information online. Then print the paperwork out at home and bring it with you to your office visit or email it to the front desk. It takes about 20 minutes to complete the form. Please make sure you have sufficient time to finish it.

  1. Fill out the additional medical history and medications online through our patient portal (you will need to request access from our staff). Please make sure you have sufficient time to finish entering your medical history.

To avoid cancellation, please arrive 20 minutes prior to your scheduled appointment time with both steps completed. If you have not completed both steps, please arrive 40 minutes prior to your scheduled appointment. The initial visit will involve a detailed history in an effort to define the factors causing your allergy or triggering your asthma. A physical examination follows. Armed with this information, our providers are able to determine what allergy tests (skin and sometimes blood tests) are needed to diagnose and confirm the severity of your allergy. Breathing, or pulmonary function tests, are used to evaluate your asthma. A treatment program can now be designed and tailored to your individual needs. The program may include environmental control, medication or allergy injections, as well as specific patient and family education relating to you.


You can request an appointment online at either the Murray or West Jordan office locations or by calling (801) 263-8700. Due to limitations in the scheduling system, our office will contact you to verify your requested appointment is available.


Cancellation Policy

Please schedule appointments that are convenient and easy for you to keep. If you find that you cannot keep your scheduled appointment because of conflict or illness, please notify us at least 24 hours prior to your scheduled appointment. We will make every effort to reschedule your appointment for another time. If you are more than 15 minutes late we reserve the right to cancel your scheduled appointment and will try to see you if our schedule permits. Please call to give us at least 24 hours notice for cancellations. Failure to do so will result in a $20 same day cancellation fee or $40 no show fee.

Telephone Policy

Please confine routine calls to office hours. We also request that you have your pharmacy call us directly during office hours for prescription refills. After hours, on weekends and holidays, your call will be taken by our answering service to be forwarded to one of our providers. On rare occasions, another qualified physician will be taking calls. If your call is not returned within 30 minutes after reaching the answering service, please let us know.

IF YOU ARE HAVING EXTREME ALLERGIC REACTION OR ASTHMA EMERGENCY, PLEASE PROCEED DIRECTLY TO A HOSPITAL EMERGENCY ROOM OR CALL 911. THE EMERGENCY ROOM WILL NOTIFY US.

Injection Policy

Although allergy serum therapy may be quite helpful for controlling your allergies, there are inherent dangers. In spite of all precautions, significant reactions still rarely occur. Most significant reactions occur on an immediate basis; therefore, you are asked to remain in the doctor’s office for at least 30 minutes following an injection.
