We appreciate payment in full at the time of your visit, but convenient payment arrangements can be made if this is not possible. Our staff will attempt to inform you of how much your insurance company will pay for a service, however, we are not responsible for this information. Your insurance benefits may not completely cover our services, and you may be responsible for the difference. If you need assistance in completing your insurance forms, we will be happy to answer your questions.
Accepted Insurance
Allergy Associates of Utah providers participate in most managed healthcare plans. It is your responsibility to contact your insurance to verify coverage. If you have questions about whether your specific healthcare plan is accepted, please call our office.
We do not participate in the following plans at this time: State of Utah Medicaid, Healthy U Medicaid, Select Share, and Molina plans. Please inquire about self-pay pricing for services if you have any of these plans.

Cancellation Policy
Please schedule appointments that are convenient and easy for you to keep. If you find that you cannot keep your scheduled appointment because of conflict or illness, please notify us at least 24 hours prior to your scheduled appointment. We will make every effort to reschedule your appointment for another time. If you are more than 15 minutes late we reserve the right to cancel your scheduled appointment and will try to see you if our schedule permits. Please call to give us at least 24 hours notice for cancellations. Failure to do so will result in a $20 same day cancellation fee or $40 no show fee.
